All index CFD fees are built into the spread. For example, the spread for S&P 500 index CFDs is 1.3.

No inactivity fee, no withdrawal fee

Specifically, FXTRADING.com charges no account, inactivity, deposit or withdrawal fees. There is a currency conversion fee, but it is low.
